Filing a DMCA Takedown Request

  5. A statement, made under penalty of perjury, that the information in your notice is accurate and that you are the copyright owner or authorized to act on the owner’s behalf.

  6. Your electronic or physical signature. 


Counter-Notification Procedure

If you believe that the content removed or disabled as a result of a DMCA notice was removed by mistake or misidentification, you may submit a counter-notification. Your counter-notification must include:

  1. Identification of the material that was removed or disabled and the location where it previously appeared.

  2. A statement, under penalty of perjury, that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.

  3. Your contact information, including name, address, telephone number, and email address.

  4. A statement that you consent to the jurisdiction of the federal court in your district (or if outside the U.S., the jurisdiction where our company is located) and that you will accept service of process from the complainant.

  5. Your physical or electronic signature.
